A nurse is in a therapeutic relationship with a client who has been making good progress. However, over the past few weeks, the client seems to be diverting attention away from himself and toward the nurse. For example, the client often asks inappropriate personal questions, and he sits too close and occasionally touches the nurse. Finally, the client has been asking for extra time with the nurse that would take her attention away from other clients. How should the nurse respond to the client? 2. A nurse has gone through the preparation and orientation phases of a therapeutic relationship with a client and has moved into the working phase. Now, the client does not seem to understand the nurse’s role. At times, the client wants the nurse to make decisions for her, but at other times, she is unreceptive to the nurse’s therapeutic suggestions. In general, the client’s listening skills are poor. How should the nurse intervene? 3. A client with paranoid schizophrenia has made a great deal of improvement. His hallucinations have abated; he is taking his medications, is compliant, and is nonviolent. As the nurse is planning his discharge and discussing it with him and other members of the health care team, the client states, “I am not ready to be discharged yet. I think I need to stay a bit longer.” How should the nurse respond to this statement? 4. The police bring a client who is homeless into the emergency department. He was panhandling in the street when a passerby made a comment to him to get lost. He lashed out and knocked the passerby to the ground. The client starts to yell, “You are not keeping me here!” and attempts to push the nurses out of his way to escape. Why would anyone prefer to live on the street rather than in a clean, warm hospital room with food provided? Evaluate the client’s actions. 1. In this situation, it is clear that the patient is confused about the role between nurse and patient. It’s important that the nurse uses simple and direct language to best maintain boundaries. It’s vital to not dance around the message you are trying to relay to your patient. The patient is asking inappropriate questions and crossing boundaries by touching the nurse and sitting close to her on purpose. Now he is asking for more time with that specific nurse. It is a good thing that the patient likes the nurse taking care of him but crossing boundaries does not help anyone in this situation.
